[:vi]Trong 25 năm, ngành nhãn khoa thế giới không ngừng nghiên cứu và cải tiến các phương pháp phẫu thuật khúc xạ nhằm điều trị triệt để tật khúc xạ (cận, loạn, viễn). Hiện tại, phương pháp ReLEx SMILE được coi là tối ưu nhất và đã mở ra một trang mới cho ngành nhãn khoa.
Phẫu thuật khúc xạ hiện là xu hướng của thế giới.
Theo tạp chí Current Opinion in Ophthalmology, mỗi năm tại Mỹ có khoảng hơn 01 triệu ca phẫu thuật laser điều trị cận, loạn, viễn bằng nhiều phương pháp như Lasik, Femto-Lasik, SmartSurfACE, ReLEx SMILE…
Mỗi phương pháp trên đều có ưu-nhược điểm riêng. Người có nhu cầu hồi phục nhanh chóng có thể thấy bất tiện nếu bị kích ứng sau phẫu thuật SmartSurfACE. Các phẫu thuật tạo vạt giác mạc như Lasik hay Femto-Lasik có thể đi kèm biến chứng về vạt.
Xét về tổng thể, ReLEx SMILE (Refractive Lenticule Extraction, small Incision Lenticule Extraction) là phương án tối ưu nhất cho bệnh nhân thường chơi thể thao, làm việc trong ngành hoạt động mạnh, cần được hồi phục nhanh và không có biến chứng về vạt hay viêm nhiễm.
Có thể nói ReLEx SMILE là bước đột phá trong ngành nhãn khoa: Không tạo vạt giác mạc, chiếu laser Femtosecond tạo ra mặt phân cách, tách phần nhu mô và rút qua đường mổ nhỏ.
Carl Zeiss, hãng chế tạo Visumax và độc quyền công nghệ ReLEx SMILE, hy vọng ngành nhãn khoa có thể tiến đến một thế hệ phẫu thuật khúc xạ không dùng dao (Lasik không dao) trong tương lai gần. Số liệu của Carl Zeiss cho biết, ngay từ khi ReLEx SMILE mới được ra đời, Liên Minh Châu Âu, Trung Quốc và Ấn Độ đã sớm áp dụng phương pháp này từ tháng Chín năm 2011. Tính đến thời điểm 2017 hiện tại, ReLEx SMILE đã được thực hiện thành công hơn 750.000 ngàn ca tại 500 cơ sở nhãn khoa trên 61 quốc gia trong đó có Nga, Mỹ, Nhật, Canada…
ReLEx SMILE – Phẫu thuật khúc xạ không có biến chứng
Theo nhiều thống kê từ 05 năm trở lại, ReLEx SMILE được phản hồi là đem lại chất lượng thị lực tốt và hầu như không có biến chứng.
ReLEx SMILE có ưu điểm là: không có biến chứng về vạt giác mạc, thời gian phẫu thuật nhanh, tia laser Femtosecond có năng lượng thấp và tần số cao giúp bảo tồn tối đa giác mạc, giảm thiểu hiện tượng khô mắt, thời gian phục hồi sau phẫu thuật nhanh hơn các phương pháp khác. Việc ứng dụng và nhân rộng phẫu thuật điều trị tật khúc xạ bằng ReLEx SMILE trên thế giới đang mở ra một kỷ nguyên mới trong ngành nhãn khoa.
Không lâu sau khi ReLEx SMILE chính thức được Cục quản lý Thực phẩm và Dược phẩm Mỹ FDA công nhận là an toàn tại Mỹ, Bộ Quốc Phòng Mỹ đã tiến hành áp dụng ReLEx SMILE trong điều trị tật khúc xạ cho binh sĩ của họ.
Phương pháp phẫu thuật khúc xạ cho người hoạt động mạnh
ReLEx SMILE rất hợp với bệnh nhân làm việc trong ngành nghề hoạt động mạnh và dễ bị chấn thương vào khu vực mắt như quân đội, cảnh sát, cứu hỏa, thể thao, xây dựng… Đây là lý do Bộ Quốc Phòng Mỹ muốn áp dụng ReLEx SMILE trong quân đội và còn nghiên cứu chuyên sâu phương pháp này cho những binh sĩ cần chiến đấu và điều khiển thiết bị quân sự.
Theo thông cáo báo chí của Carl Zeiss, ca ReLEx SMILE đầu tiên đã được các bác sĩ quân y thuộc Hải Quân Hoa Kỳ thực hiện tại California.
Sĩ quan, bác sĩ John B. Cason đã thực hiện phẫu thuật ReLEx SMILE tại Trung tâm Phẫu thuật Khúc xạ Hải quân tại San Diego. Tổ nghiên cứu ứng dụng phẫu thuật khúc xạ của trung tâm sẽ theo dõi khả năng thị lực của bệnh nhân đã được phẫu thuật ReLEx SMILE “trong điều kiện khắc nghiệt và căng thẳng”.
“Sự kết hợp giữa Quân Đội Hoa Kỳ và Carl Zeiss đánh dấu một mốc mới cho ReLEx SMILE. Từ bỏ kính gọng và kính áp tròng có thể tăng cường chất lượng cuộc sống cho người bình thường. Nhưng với binh sĩ, những người thường xuyên hoạt động trong môi trường nguy hiểm, thì việc cải thiện thị lực mang tính chất sống còn và giúp nâng cao tỷ lệ thành công ở các nhiệm vụ quân sự,” Jim Mazzo, giám đốc phụ trách thiết bị nhãn khoa toàn cầu của Carl Zeiss nhận định.
ReLEx SMILE – áp dụng được cho khoảng 99% các trường hợp tật khúc xạ
Các bệnh nhân có độ cận thị cao đến trên 10 đi-ốp, có độ loạn thị từ 0,5 cho đến 5 đi-ốp đều áp dụng được ReLEx SMILE. Những mắt có tình trạng rất kém cũng có thể được cải thiện bằng phương pháp này.
ReLEx SMILE được đưa vào nước ta từ năm 2012 đã mở ra cơ hội điều trị an toàn cho rất nhiều người. The Naval Medical Center San Diego is scheduled to perform first procedures in the coming weeks. Following the recent U.S. FDA approval* of ReLEx SMILE for the treatment of myopia, or nearsightedness, all three branches of the DoD, the Army, the Navy and the Air Force, have plans to offer SMILE for eligible service members. This will include an extensive evaluation of SMILE to correct myopia and astigmatism under some of the most visually demanding conditions. The DoD has an expansive refractive surgery program with over 20 dedicated centers around the world which performed over 40,000 refractive surgery procedures last year.
“Dependence on eyeglasses or contact lenses can be a challenge for many people in their daily lives. The conditions that nearsighted patients face in the performance of military duties can be life-threatening if their vision is compromised,” says Jim Mazzo, Global President Ophthalmic Devices at Carl Zeiss Meditec. “The expectations of surgeons and patients who serve in the military are exceptionally high. These first cases by Dr. Rivers represent the first step towards adoption of SMILE across all three branches and will enable their surgeons to offer the newest, most advanced refractive laser technology to service members who face some of the most demanding situations requiring perfect vision.”
“We are excited that U.S. service members can now be treated with this innovative technology and that we have a new refractive surgery option to help increase the safety and the performance of the service men and women who are called to go into harm’s way,” said Army Col. Bruce Rivers, M.D., Director of the Warfighter Refractive Eye Surgery Program and Research Center at Fort Belvoir Hospital. “The visual requirements for military personnel are demanding. Myopic correction with eyeglasses or contact lenses restricts some of the best and brightest from various roles in the military and causes difficulties in deployed environments. SMILE is a breakthrough surgical vision correction option which can be life-changing for many and life-saving for first responders and for those in dynamic roles.”
ReLEx SMILE, a high-precision procedure which was recently approved by the U.S. FDA, is an innovative minimally-invasive approach to myopia correction that is only available on the VisuMax® femtosecond laser from ZEISS. In the SMILE procedure, surgeons correct patients’ myopia using the VisuMax femtosecond laser to create a precise disc-shaped lenticule within the cornea, which is then removed by the surgeon through a small incision, also created by the laser.
“SMILE has shown excellent reproducibility and predictability in the U.S. FDA clinical study, even with severe corrections. SMILE can help improve the readiness for our active duty members as it can eliminate reliance on vision correction methods such as eyeglasses or contacts,” continues Dr. Rivers. “Plus SMILE is a laser vision correction option that may result in greater strength and stability for the eye compared to alternatives. Patients typically experience minimal discomfort and can rapidly return to daily activities.”
“For over 20 years, the DoD has been a pioneer in refractive surgery, independently evaluating and then offering the best refractive technology to the people that we entrust to defend our country,” says Steven C. Schallhorn, M.D., founder of the U.S. Department of Defense refractive surgery program and Chief Medical Officer for Global Ophthalmic Systems at Carl Zeiss Meditec. “This first SMILE procedure is another important milestone which I believe will usher in a new direction for the DoD refractive surgery program.”
Over 700,000 SMILE procedures have been performed internationally since its introduction in 2011 with over 1000 surgeons regularly performing the procedure. Now with the U.S. FDA approval, SMILE, the latest refractive technology from ZEISS, can be used by doctors at the U.S. Department of Defense to continue providing the most advanced medical care to U.S. service members.
ZEISS is also conducting an IDE trial in the U.S. on astigmatic myopia to further broaden the spectrum of SMILE for more patients.

For use in the reduction or elimination of myopia -1.00 D to -8.00D, with ≤ -0.50D cylinder and MRSE -8.25D in the eye to be treated in patients who are 22 years of age or older with documentation of stable manifest refraction over the past year.
